Address The Outer Envelope

  • Front, top-left: Your return address (the Authorized Account Holder’s verified residential address).
  • Back, mid-center (not top): Your Mail-In Code exactly as shown on screen (correct format only).

Fill out the 4″ x 6″ Request Card

Top-left (in this order):

  1. Full name (exactly as shown on your Player Account)
  2. Email address registered to your Player Account.
  3. Mail-In Code (exact on-screen format)
    Return residential address as listed on your government-issued ID associated with your Player Account.

Top-right: The date you prepared the Request Card.

Below the info above, write this exact statement:

Insert the unfolded Request Card into the #10 envelope.

Final Checks Before Mailing

  • LIMIT: 1 Request Card per outer envelope.

  • No status updates are available during review.

  • H5C isn’t responsible for lost, late, incomplete, invalid, unintelligible, or misdirected requests.
  • H5C may update the Mail-In process. Requests are governed by the Official Rules in effect by postmark date (there is a grace period for mail already in transit).
  • Tampering/automation/outsourcing the handwritten requirements can lead to disqualification and account suspension/termination.
  • In any dispute about identity or format compliance, H5C’s decision is final.

Common Reasons For Disqualification

  • Using multiple ink colors or non-handwritten text.
  • Illegible handwriting (Sponsor decides legibility).
  • Wrong format/placement (e.g., Mail-In Code not on the back mid-center of the envelope; missing required items on the card; card not landscape; writing on both sides; card folded).
  • Envelope not #10, or lined/colored, or mailed from a different state than your verified residential address.
  • There is more than one Request Card in the envelope.
  • Expired or someone else’s Mail-In Code.
  • Name or address that doesn’t match your Player Account/government ID.
